Mount Sinai Adolescent Health Center’s Project IMPACT (Improving Access to Care and Treatment) service provides treatment, support and care for HIV + young people ages 13-24, including comprehensive medical services, case management and mental health counseling for any young people and their families impacted by HIV/AIDS.
Each patient is assigned a social worker and doctor who work together as a team to provide high quality holistic health care services. We take a patient centered approach, with the understanding that each individual has different needs and will require different levels of services. The resources offered by Project IMPACT are specifically geared to enhance patients’ life skills, particularly as they transition into adult care, by helping them reach and maintain undetectable viral loads, adhere with medication regimens and comply with medical appointments – all of which are essential to living long, healthy and productive lives.
